The Garage, a population health management technology company providing patient-centered solutions to stakeholders across the industry, today announced a new partnership with California-based National ACO. The organization has selected The Garage’s SaaS-based population health management platform, Bridge, to streamline data collection and reporting across physician practice locations in several different states.
National ACO is actively engaged in population health through their participation in the Medicare Next Generation ACO Model program. With provider networks in multiple states, including California, Nevada, Tennessee and Pennsylvania, National ACO was looking to partner with a single source provider to aggregate clinical and administrative data from multiple sources, providers, and systems, to provide actionable reporting to management, physicians and staff in the different locations.
